Go to the Patient Info tab. All the fields where you have to enter information appear in orange. If you do not have and cannot get the requested information, there is always a choice such as “Unknown”. Please try to add as much of the requested information as possible. Missing data makes it difficult to interpret the results. Once you enter the requested information the orange will disappear.
Treatment Status:
This should read “Active”. Later when they finish or discontinue treatment, you will change it by clicking on the field. Beside “Active” there are three other options. Finished means the client has completed treatment. The other choices are “D/C NOT Treatment Related” and“D/C Treatment Related”. The former is if they stopped because they moved, something changed in their life that they couldn’t come for sessions, etc.
The latter is if the client stopped neurofeedback because they felt it was not helping, was causing adverse effects or any other indication of dissatisfaction. If this is the case, in the field directly below, please enter a brief description of why they stopped as it will help us understand and improve the treatment.
Initials, Year of Birth and Gender:
These were created when you created the client file.
ACE’s Questionnaire:
ACE’s is the Adverse Childhood Events Questionnaire. Go to the ACE’s tab with separate questionnaires for children and adolescents or adults. Instructions for how to complete this questionnaire are on the ACE’s tab. Once you checked all items that were Yes, return to the Patient Info tab.
Type of Client:
Select the appropriate category for this client. “Medicaid” can include clients who are not formally enrolled in Medicaid but would be eligible for Medicaid. Other choices are Pro-bono and Fee for Service.
Choose the best option.
Client in Child Services?
If the client is a child, select the type of service they are receiving. Select None if the child is not receiving any special services. Select Client is Not a Child for an adult client.
Race/Ethnicity:
Choose the best option.
Receiving neurofeedback?:
This option is to use the RTS to compare results for clients who didn’t get neurofeedback. If the client will be getting neurofeedback, choose YES NFB.

Informed Consent Signed?:
You must have an informed consent that covers both neurofeedback and that data is being collected. An example of what that form can look like is here. This form has been vetted by the legal resources at many agencies as complete and appropriate. You and your agency may elect to create your own form. Whichever you choose, please make sure to acknowledge that it has been done by selecting “Yes”.
Did Any Of These Happen?:
The next ten items ask about events that might have happened to the client in the year before starting neurofeedback. You will be asking the same 10 questions after every 20 sessions of neurofeedback.
Current Stresses:
Go to the Current Stresses tab. The instructions are on the tab. Once you have checked all items that are Yes, return to the Patient Info tab.
How well are you coping?
This asks the client to rate on a 1 to 5 scale how well they are coping with their current stresses. Select the score.
Medications?
This simply asks whether the client is currently taking medication for emotions or attention and at later points in time whether the medication has been increased, decreased or they are no longer taking medications.
How helpful is…?
This asks the client to rate the five most commonly used therapeutic services they currently may be receiving. Enter “0” if they are not receiving that type of therapy.
Opinion of neurofeedback?
This is left blank until after they have had 20 sessions. After 20 sessions, ask the client for their opinion in their words for both positive and negative comments.
Collect Information After Every 20 Sessions
After every 20 neurofeedback sessions remember to ask again about the items in the Test Results, Client Events and Client Ratings sections of the Patient Info tab. These cells will have turned orange when it is time to get the information.
